Covid-19 Vaccinations ages 12-15: Monday 17th January 2022

Dear Parent/Carer 

 Sussex Community NHS Foundation Trust will be offering doses of the COVID-19 vaccine to all 12–15-year-olds in your child’s school on the 17th January 2022 . At these sessions children will be able to receive, if eligible, either their 1st or 2nd dose of the COVID-19 vaccine.  

 Please be aware that all parents/carers will have to complete a consent form to receive a vaccine on the day.  

This can be done by following the instructions in the attached letter.  

 As well as the above, please be aware:  

  1. If your child cannot make the date in school weekend clinics are available –with more to be added – these can be booked via our booking system; Cinnamon. You will receive further information about how to book into these clinics if your child does not receive the vaccine during the school visit, but if you have any questions about how to do this call the Vaccine Enquires Team on 0800 433 4545 (open 9am – 4pm, Monday to Friday, excluding Bank Holidays);  
  2. Further clinics are currently available via the National Booking Service which anyone can book on to using this link: http://www.nhs.uk/covidvaccine
