Re: HPV Vaccination – Monday 22 September 2008

Further to our recent letter regarding the annual national HPV vaccination programme for girls aged 12-13 years, I am writing to let you know that we have now received notification of the vaccination schedule.

The vaccination will consist of three injections, delivered over the coming months on the following dates:

Monday 22 September 2008

Monday 17 November 2008

Monday 23 March 2009

If you would like your daughter to receive the HPV vaccination, please complete the enclosed consent form and return it to Student Services by Wednesday 17 September.
