Re: HPV Vaccination – Monday 21/ Tuesday 22 September 2009
Further to our recent letter regarding the annual national HPV vaccination programme for girls in Years 8, 10 and 11, I am writing to let you know that we have now received notification of the vaccination schedule.
The vaccination will consist of three injections, delivered over the coming months on the following dates:
First injection: Monday 21/ Tuesday 22 September 2009
Second injection: Monday 9/ Tuesday 10 November 2009
Third injection: March 2010- date to be confirmed.
If you would like your daughter to receive the HPV vaccination, please complete the enclosed consent form and return it to Student Services by Wednesday 16 September.
